Prior to each academic year, families will receive hard copies of forms to complete. Fillable forms can also be downloaded below and mailed/brought to the office or faxed to 847-295-2457. If you have any questions about the school forms, feel free to call the school office at 847-295-8338.
According to the IDPH State mandates, the following are Forest Bluff's Health Requirements for the 2022-2023 school year:
Health Exam must be completed for new students, students entering Illinois school for the first time, Pre-K (1-4 years old), K (5-6 years old), and 6th grade (11-12 years old).
Dental Exam must be completed for K, 2nd (7-8 years old), and 6th.
Eye Exam is due for new students, students entering Illinois school for the first time, and K.
